1. Contact Coach directly to coordinate private lesson scheduling
2. Submit Player Information and Private Date/Time/Coach Info
3. Submit Parent and Emergency Contact Information
4. Submit Medical Insurance Information
5. Submit Payment Method for Lesson (NOTE: We are now requiring one parent to book and pay for group lessons up front. That parent can coordinate getting reimbursement back from the other player that is participating in the group lesson).

Private Lesson Booking Rules:

• You must be a current player of MVA (during the club season). Or not affiliated with a club team.
• Club coaches may not conduct lessons with a member of their team (during the club season)

Contact Coach:

Before submitting the private lesson form, you must contact the Coach directly that you are interested in running a private with.   Below are the MVA Coaches currently running private lessons and their contact info.  Coordinate the private with them first. After you have an agreed upon date and time, then submit the registration for the private lesson.
